Abstract

Effective patient management is essential for improving clinical outcomes and optimizing healthcare service delivery. This study explores the application of the Transition Probability Matrix (TPM) as a tool for analyzing and enhancing patient attributes within healthcare systems. By modelling patient movements across key hospital states such as admission, treatment, and discharge, TPM enables a data-driven assessment of patient flow and the identification of operational inefficiencies. The paper emphasizes a bibliometric analysis of TPM to demonstrate its research significance and application trends. The findings offer insights into optimizing patient care, workload allocation, and process bottlenecks. Additionally, the study discusses integrating TPM into healthcare decision-making frameworks to improve patient experience, reduce waiting times, and strengthen capacity planning. Overall, the proposed approach supports hospital managers and policymakers in adopting a more systematic, patient-centred strategy to enhance healthcare efficiency.
